In Operation: Anchorage là phiên bản mở rộng tập trung vào lối chơi Combat Stealth và để chiến thắng, người chơi buộc phải vận dụng và phát huy tính sáng tạo và sự khôn ngoan trong chiến đấu. Trong In Operation: Anchorage, người chơi sẽ đặt chân lên vùng đất phía bắc băng giá quanh năm. Nơi đây, quân đội đỏ của Trung Quốc (Chinese Red Army) xuất hiện ở khắp mọi nơi và nhiệm vụ của người chơi trước tiên là tạm chiếm những khu vực núi cao xung quanh và tổ chức tập kích các căn cứ của quân Trung Quốc.
Người sẽ sẽ phải sử dụng rất nhiều kỹ năng chiến đấu cơ bản cùng với một vài công cụ mới trong các nội dung có thể tải về được. Trong đó, người chơi sẽ có dịp điều khiển một nhóm quân mang tên Strike Team và được sử dụng những món vũ khí, áo giáp và đồ chơi độc đáo và mới lạ. Bản mở rộng cũng bao gồm nhiều nhiệm vụ và bản đồ chơi mới để người chơi thám hiểm. Người chơi có thể trải nghiệm những nội dung mới này ở bất cứ trình độ nào, một khi đã bước ra khỏi Vault. Bên cạnh đó, người chơi còn được tiếp cận và sử dụng những công nghệ chiến tranh tối mật mà cả hai bên đã sử dụng trong cuộc chiến.
Nguồn gốc Operation: Anchorage bắt nguồn từ việc team phát triển Fallout 3 muốn thử thực hiện những việc mới mẻ, phát huy tính sáng tạo của các thành viên.

In Operation: Anchorage the player will find themselves able to re-live the famous liberation of Anchorage from Fallout lore -- inside a simulation similar to one found along the main quest of Fallout 3. Once the player finds their way into the simulation, they'll be stripped of their resources and have to survive within the rules set up by the simulation's creators.

The Chinese red army is everywhere, and the player will first have to secure the surrounding mountain side and then fight their way into the Chinese base. The player will have to use a lot of their standard combat skills, along with several new tools that will only be available in the downloadable content. These include interactive Strike Teams under the player's command and unique armor, weapons, and other exotic gadgets...
